* All participants of Carolina ARMAGEDDON! must read and sign the disclaimer/parent permission form. This form will need to be completed for each event.

If a participant does not have the form with them, they will be asked to complete it at the event.

If you are under the age of 17, you must be accompanied by an adult to the LAN party and must have the "Parent" section of the form completely filled out.

**Due to legal resons, we must have Valid participant names. At the LAN you will be required to present photo ID, and this must match your Registration. Failure to properly complete the Registration form will result in the Registration being rejected, till such time as the Registration is successfully updated.
